The Department of Geriatric Rehabilitation of A.S.P. Ragusa has the task of coordinating and integrating all the activities of rehabilitation, intermediate rehabilitation care and rehabilitative social care. This is achieved by creating clinical paths of continuing care, which are based on criteria of uniformity, promptness, efficiency, efficacy and appropriateness. The objectives are to ensure a maximum recovery of the autonomy or to prevent secondary and tertiary damages, and to limit the reduction of participation, in order to help disabled persons in reaching a better quality of life and facilitating, where possibile, social and work integration.
It ensures an adequate rehabilitation process for all the people who need it. Moreover, it represents an hub for Clinical Governance.
The Department of Geriatric Rehabilitation is highly integrated, from an organizational standopoint, with all the hospital, territorial and private-accredited facilities of the area, according to criteria of efficiency, efficacy and appropriateness.
It ensures continuity between the hospital and the territory, coherently with the needs of the patients, by organizing clinical paths in the different settings: within and outside the hospital, in the territory, as well as in healthcare and social settings.
In each territorial area, the main organizational tasks of the Department of Geriatric Rehabilitation consist in:
- creating facility projects as well as rehabilitation projects, sorted according to the scope and to the gravity of the related health need;
- creating connections between the different actors of the rehabilitation network;
- planning and creating the missing elements of the paths;
- monitoring and preserving the minimum accreditation requirements (risk management for the criticalities of facility, organizational and process resources);
- sharing, between the different actors of the services system, a code of ethics for the safeguard of highly critical situations and for a fair use of shared resources.
The strategic principles are the following: the Department is the governance operative body and integrated care pathways are both instruments and indicators of the quality and the safety of the care provided. Moreover, the medical organization of rehabilitation is focused on the citizen-patient and his/hers well-being and health. Finally, services are consequently organized, by moving from an organization which is based on providing services to one which is based on the outcomes, that are the results that have to be achieved on each patient.
These principles are fulfilled in patient’s management and in the provision of interventions which follow definite rehabilitation plans, within a specific individual rehabilitation project.
The Department consists of hospital and territorial divisions.
The hospital divisions are:
- Advanced Operative Unit of Recovery and Functional Rehabilitation, Ospedali Riuniti of Comiso/Vittoria;
- Advanced Operative Unit of Recovery and Functional Rehabilitation, Ospedali Riuniti of Scicli/Modica;
- Advanced Operative Unit of Geriatrics, Modica;
- 2 Basic Operative Units of Geriatrics;
- Departmental Basic Operative Unit of Recovery and Functional Rehabilitation, Ospedali Riuniti of Ragusa;
- Basic Operative Unit for Post-Acute Phase Long Hospitalization, Comiso;
- Basic Operative Unit for Post-Acute Phase Long Hospitalization, Scicli.
The territorial divisions are:
- 3 R.S.A.s (Residential care homes) in Comiso, Ragusa, Scicli and 1 S.U.A.P. (Special Unit for Continuing Care) in Ragusa;
- Rehabilitation Outpatient Clinics (Vittoria/Comiso/Ragusa/Modica/Scicli/Pozzallo/Ispica);
- externalized Home Rehabilitation Service (A.D.I.);
- Rehabilitation provided for by ex article 26 (C.S.R.s and C.M.S.s), in the area.
