The Patients Registry Office ensures all the population, who is divided according to the District and to territorial areas, the assignment and the change of general practitioners and pediatricians, in compliance with the regulations in force.

From the 1st of July 2021, the new online change service for the choice of a general practitioner is available at A.S.P. Ragusa, together with the related user guide, in order to support users in choosing a general practitioner.

In order to access to the “Online change of General Practitioner” service, it is necessary to register and access the “Online Services” section of the website www.asp.rg.it

In the “Online Services” section, click on the desired service, in this case “Online change of General Practitioner”. The online service can be also used on mobiles.

In order to register, users are required to enter their data: tax code, mobile number and email.

New registration and choice of a doctor

Entitled citizens can choose a doctor for themselves, their spouses and their minor family members; a proxy is required in the case of family members over the age of majority. Doctors have to be chosen among those registered in patient’s Territorial Area of residence, within the limits of the maximum number of patients for a single doctor. Required documents:
  • Application form (Form A)
  • Identity document
  • Health insurance card
  • Self-certification of residence (Form B)
  • S1 Form (ex E120 or E121) for holders of CEE pension
  • S1 Form (ex E106) for posted workers and (ex E109) for family members of workers who reside abroad
In the case of choice by means of proxy, the delegate has to submit the abovementioned documents accompanied by:
  • Proxy form (Form D)
  • A photocopy of the identity documents of the delegator and the delegate
Italian citizens who are registered to A.S.P. 7 of Ragusa, from a period of at least three months and not greater than one year, for the following reasons:
  • Study
  • Work
  • Healthcare
Required documents:
  • Certificate of cancellation from doctor’s patients list, issued by the Local Health Authority of residence
  • Declaration of domicile registration in the territory of A.S.P. 7 (Form C)
  • Identity document
  • Health insurance card
  • Documents pertaining the reasons for domiciliation
  • Self-certification (Form B)
In the case of choice by means of proxy, the delegate has to submit the abovementioned documents accompanied by:
  • Proxy form (Form D)
  • A photocopy of the identity documents of the delegator and the delegate

Registration is provided for stays which are longer than three months, in the following cases:
  • Salaried employees or self-employed workers in Italy;
  • Family members (non-EU citizens included) of salaried employees or self-employed workers in Italy;
  • Family members of Italian citizens;
  • Residents who hold a certificate of “permanent residence”, accrued after at least 5 years of residence in Italy;
  • Unemployed persons registered to job seeker's lists, who are in a involuntary unemployment status, after over a year of duly substantiated working activity;
  • Persons who hold one of the following EU forms: S1 (ex E106, E109, E120, E121)
Required documents:
  • Identity document of the country of origin
  • EU citizens form (Form E)
  • Tax code
  • Employment contract, in the case of salaried employees
  • VAT identification number or self-certification, in the case of self-employed workers
Registration is provided for those who are in possession of a valid residence permit issued for the following reasons:
  • Salaried employment and/or self-employment
  • Expected occupation
  • Family reasons, family members aged over 65 whose arrival in Italy is prior to 5.11.2008 included
  • Political and humanitarian asylum
  • Care of minors
  • Pending adoption
  • Foster care
  • Application for citizenship
  • Health reasons (residents who arrived in Italy for “medical care” excluded)
  • Residence card holders and long-term residents
  • Non-EU family members of EU citizens who are registered to Regional Health Service
  • Pending regularization
  • Unaccompanied foreign minors who are in Italy, regardless of the possession of a residence permit
  • Study reasons, for adults who are already registered for mandatory reasons
  • Convicts and inmates
  • Permits for legal reasons
  • Religious reasons for religious who carry out a working activity
  • Stateless person status
  • Study reasons, in the case of students who carry out a working activity
  • Elective residence, for holders of an Italian contributory pension

  • A valid residence permit or its renewal application
  • Non-EU citizens form (Form F)

  • Documents that justify the presence in Italy (such as employment contract, VAT identification number, unemployment status certification etc.)
Foreign (both EU and non-EU) citizens who are in possession of a residence permit with a duration of over three months, issued for the following reasons, are entitled to voluntary registration:
  • Elective residence
  • Religious reasons
  • Foreign citizens included in volunteer programs
  • Family members aged over 65 arrived in Italy for family reunification after 5th November 2008
  • Other categories excluded from mandatory registration
